ATTENTION NEW PATIENTS 

  •  Go to"LINKS" to download Registration, Health History and Privacy Notice forms to print and complete prior to your office visit
  • Bring the above forms, your insurance information, ID, and any prior medical records you may have to your first visit. 
  • Immunization records are required for patients less than 18 years old and patients having a complete physical for school, sports or camp

LSFP RECOMMENDS

For after hours emergencies like a minor illness or injury that cannot wait, we recommend the LMG Immediate Care Center in Sterling.  We share the same electronic medical record system so the providers there will have access to your medical history and medication lists.  Identify yourself as an LSFP patient and you will only be charged your regular co-pay!  Go to "LINKS" to view their website for more details.

We routinely screen for the HPV virus when you have a routine Well Woman exam and pap smear.  Not all insurance plans cover this test even though it is recommended by the CDC and your healthcare provider.  We recommend you contact your insurance company to verify coverage prior to making an appointment for your annual exams.

Are your canker sores a pain in the mouth? If they're large, last longer than a week or make it too painful to eat, see your family doctor.
